Provide prescribed medications, and other pharmaceuticals, compound medications according to professional standards and practices. Advise/consult with patients, families, medical and nursing staff.
1/ Review patient's drug regimen on a concurrent basis and verify the physician's original order for appropriateness prior to dispensing.
2/ Communicate any suggested changes to the prescribing physician.
3/ Prepare, check and dispense outpatient prescriptions issued by physicians.
4/ Prepare, package, dispense and deliver unit dose medication in accordance with standard procedures and techniques for hospitalized patients.
5/ Provide patient education, counselling regarding drug therapy, the effects, dosage, route of administration of the drug treatments, drug related interactions, particularly for those patients who require complex drug therapy;
6/ Prepare drug formulations, strength, dosage forms, and packages that are useful in the care of patient and not available commercially but approved by the Pharmacy and Therapeutics committee.
7/ Prepare and quality-check all sterile medications under special conditions (e.g. intravenous medications, eye drops, etc.).
8/ Monitor every stage of medication therapy to improve all aspects of delivery and reporting of patient side effects and/or medication errors.
9/ Order pharmaceutical supplies, and drugs, maintaining stock and storing and handling it properly.
10/ Maintain and control of the floor stocks of drugs kept in the wards and nursing units.
11/ Perform routine inspection of all drug storage area within the hospital in order to ensure that no outdated or unusable drugs are present.
12/ Respond to medication-related queries from healthcare professionals within the hospital.
13/ Participate in evaluating new medications and recommend new medicines that are the safest and most effective for individual patient needs.
14/ Keep records such as pharmacy files, patient profiles, charge system files, inventories, inspection reports, etc.
15/ Work with other healthcare professionals to plan, monitor, review, or evaluate the quality or effectiveness of drugs.
16/ Protect patients and technicians by adhering to infection-control protocols.
17/ Maintain safe and clean working environment by complying with procedures, rules, and regulations.
18/ Maintain pharmacological knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
19/ Supervise and check the work of less experienced and less qualified staff.
20/ Liaise with physicians, nurses and other healthcare professionals to ensure the delivery of safe, effective and economic drug treatment.
21/ Maintain Confidentiality at all times.
22/ Perform any other duties that are within the scope of his/her skill necessary to the operation of the pharmacy department.
